Matrilwood
Picture this in your head, you're wondering through the woods you and your team-mates have been seriously injured, and the monsters are hot on your heels. One of your team mates has been bitten and will die if the infection isn't treated soon. You find a tavern, hope at last, but you know that your chances are still slim as the monsters will follow you inside. As you enter the building you are hear this. Have you ever experienced really bad choice of score in video games where the music just kills it?
CrypticQuery
As you enter the building you are hear this. That's like having "Another One Bites the Dust" or "Staying Alive" playing at a funeral! I've actually played games with mostly excellent music now that I think about it ^^
DoctorAllosaurus
Monster Party's game over music immediately comes to mind. Keep in mind that the game over screen has a pool of blood with skeletons floating in it. Edit: Here's a pic, its kinda small though...
Nova
You are running through the basements of a giant building after having just recovered the information you need to stop the covenant invasion. You work for hours running as fast as you can. Finally you see a clearing and reach it. Your team is safe. Everyone is happy to made it out alive. Then right as you think the level is over. BAM! The sexually attractive female in the team gets killed with a sniper headshot. In combination with the music I felt so sad D:

May not be fitting, but that's a pretty good 8-bit tune! :)

One game that was really bad for this was Champions of norrath. This was due to what I think was an error, because the town music would play none stop. So you are in a city in the trees when this plays. That's fine. It doesn't really work when a giant orc warlord appears and you have a climactic battle to the death. After defeating him, you enter a cave full of orcs, the music plays again. Then you break the seige, music plays again, then finally you enter the spider tombs, guess what the music in there is? It's a long game (I'd guess 30 hours, maybe more) and there are about 4 acts, each with 1 music track and a couple of ambiant ones. Hence why I say it's a glitch because the OST has about 15 tracks. I can only recall hearing about 5 or 6 the entire game. If you think I'm exagerating, here's a song that plays when a town is being eaten by giant ants.
